Job Title - Remote Nurse Practitioner, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day
Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education.
DESCRIPTION OF COMPETENCIES
A. Operational Duties
• Conducts in home and telehealth assessments for patients with complex needs, including formulating a diagnosis,
ordering and interpreting diagnostics, and delivering appropriate care in line with patient preferences and goals
of care
• Prescribe new medications and perform medication reconciliation
• Demonstrate sound clinical judgement and evidenced based practice in symptom management and end of life care
• Facilitate family meetings, goals of care conversations, and ACP discussions
• Leads oversight of care plans in conjunction with the rest of the clinical care team
• Advocates for the patient and caregiver in collaboration with other care providers to keep care coordinated
and consistent with patient goals
• Serve as a link between health and social care services and community to improve access to palliative care
and quality of care
• Work closely with interdisciplinary team to address changes in patient needs or symptoms
• Educates patient/family on trajectory of illness, use of medications, assesses disease understanding
• Participate in weekly IDT meeting with care team
• Excellent teamwork, communication and leadership skills
• Documentation as required

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $123,576.00 - $197,400.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
