Description

 

Provides rehabilitative care and therapeutic services to residents with diverse physical needs. Collaborate with other healthcare professionals to enhance the physical well-being, independence, and quality of life for our residents.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide rehabilitation services under the direction of a physician.
  • Instruct, prepare, and reassure resident of treatment being performed.
  • Conduct initial and ongoing physical assessments of residents to develop individualized treatment plans that address mobility, strength, and balance issues
  • Develop and implement effective, evidence-based treatment plans tailored to each resident’s specific physical needs, functional goals, and preferences.
  • Provide physical therapy interventions, including but not limited to strengthening exercises, balance training, joint mobilizations, gait training, and manual therapy techniques.
  • Monitor and ensure resident safety during treatments.
  • Work collaboratively with the interdisciplinary team of nurses, physicians, occupational therapists, speech therapists, and other healthcare providers to ensure comprehensive care.
  • Educate residents and their families on proper body mechanics, mobility aids, and strategies to improve daily functioning, independence, and overall health.
  • Ensure the highest standards of safety and professionalism in providing care, following all facility protocols, infection control practices, and ethical guidelines.
  • Provide care appropriate for the patients served based on demonstrated knowledge of the principles of growth and development over the life span.
  • Assess and interpret data on the resident’s status to identify patients’ requirements relative to their age specific needs and provide care needed as stipulated in departmental policy.
  • Maintain records associated with resident's treatment.
  • Record treatment results and progress notes accurately and legibly in resident charts.
  • Communicate information to the director and physician on resident’s treatment progress as directed.
  • Assist in maintaining logs, charge tickets, and reports.
  • Assist in maintaining stock levels of supplies and required equipment.
  • Ensure inventory is adequately stocked with supplies necessary for efficient operation.
  • Perform the sterilization of equipment and work area.
  • Perform minor and preventative maintenance on equipment and ensure equipment is properly stored after use.
  • Report equipment problems to Rehabilitation Director.

OTHER SIGNIFICANT REQUIREMENTS:

  • Maintain confidentiality on a daily basis.
  • Work well with other employees.
  • Perform other functions and tasks as assigned.
  • Follow standard precautions when providing direct patient care.

Qualifications

EDUCATION: Graduate of an accredited degree program in physical therapy required.

EXPERIENCE: One year experience as a licensed physical therapist preferred.

PERSONAL JOB-RELATED SKILLS: Knowledge of physical therapy procedures and equipment. Effective oral and written communication skills.  Familiar with operating computers and ability to utilize computer systems. 

LICENSURE, REGISTRY, CERTIFICATIONS: Current Physical Therapy License in the state of Texas required. Current CPR certification required.

PHYSICAL AND MENTAL REQUIREMENTS: Duties of the position require extended periods of standing and walking. Repeated bending, stooping, and reaching. Pushing and pulling up to 150 pounds and carrying up to 75 pounds. Lifting to 75 pounds and heavier lifting in transferring or supporting who may suddenly become weak or helpless. The ability to comprehend and follow written and verbal instructions.
